Step 1 - Important Information - Please Read

Vic Tesolin Woodworks – INDIVIDUAL WAIVER OF LIABILITY

“Release of Liability, Waiver of Claims, Assumption of Risks, and Indemnity Agreement”

W A R N I N G! WOODWORKING IS RECOGNIZED TO BE AN ACTIVITY WITH A HIGH LEVEL OF RISK IN TERMS OF PERSONAL INJURY AND/OR DEATH. VIC TESOLIN WOODWORKS ACCEPTS NO RESPONSIBILITY FOR ANY ACCIDENT, INJURY, DEATH, LOSS, OR DAMAGE TO PERSONAL PROPERTY RESULTING FROM THE USE OF ITS PROPERTY FOR SUCH ACTIVITIES. ANY PERSON OR GROUP OF PERSONS ENGAGING in THESE ACTIVITIES DO SO AT THEIR OWN RISK.

By signing-up for in-person session with Vic Tesolin Woodworks (“VTW”), you agree to the following:

  • You understand that Activities covered in sessions with Vic may be physically strenuous and you voluntarily participate in them on you own with full knowledge that there is risk of injury, loss, and/or death. You understand that you are responsible for your well-being.

  • IN CONSIDERATION OF VIC TESOLIN WOODWORKS permitting me to utilize its property, THE UNDERSIGNED does, for himself or herself or themself and his or her or their heirs, executors, administrators and personal representatives, hereby remise, release and forever discharge Vic Tesolin Woodworks and its successors, assigns, staff, employees, and volunteers, from all manner of action, causes of action, suits, claims or demands of whatsoever nature or kind against Vic Tesolin Woodworks or its successors, assigns, staff, employees, or volunteers, which the undersigned, his or her or their heirs, executors, administrators and personal representatives had, now have or may hereafter have by reason of personal injury or death or loss or damage to property arising out of the participation of the undersigned in such activities or instructional program at 18 Dundee Drive, Stoney Creek, Ontario, L8G 4K4 on the first date of activity and any future dates of activity.

  • You understand that you may ask any questions or request further explanation or information about the procedures at any time before, during, and after the virtual session.

By agreeing to this, THE UNDERSIGNED hereby acknowledges that as of the date hereof, he or she or they is eighteen (18) years of age or over, read this document in its entirety, that you understand its contents, meaning, and effect, and that you have agreed to its terms completely and voluntarily. You also acknowledge by your completed registration that you accept the allocation of risks and responsibility as between VTW and yourself set forth in this agreement.
